This post‑acute care facility, offering long‑term, short‑term, and geriatric services, seeks a full‑time Physical Therapist to join our engaging in‑house rehab program.
Responsibilities
Conduct effective screening and evaluation of patients with physical functioning disorders and wound care, developing appropriate care plans adhering to regulatory and clinical standards.
Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients in accordance with practice requirements.
Administer procedures within the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive devices, and wound care.
Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Generate comprehensive discharge summaries ensuring compliance with regulations.
Qualifications
Physical Therapy license required.
Open to all experience levels. New graduates welcome.
