Position Summary

The Nurse Practitioner performs Face-to-Face Encounters in patients' homes, nursing homes, and inpatient facilities to determine continued eligibility for Medicare hospice benefits. The role collaborates with the People’s Hospice Medical Director and involves coordinating patient care and documentation. Essential Duties & Responsibilities

Conduct Face-to-Face Encounters for Medicare Hospice Beneficiaries starting their third benefit period and beyond. Collaborate with RN Case Manager or RN Patient Care Coordinator to schedule and coordinate encounters. Consult with team members on a PRN basis for clinical information related to patients and families. Review patient records and pertinent medical information prior to encounters. Document clinical findings in the Electronic Medical Record (EMR) or on hospice-provided forms. Provide clinical findings to the certifying physician for continued eligibility per Local Coverage Determination (LCD) criteria. Facilitate palliative care for patients, collaborate with the Palliative Care Medical Director and team members. Prescribe or recommend drugs or other forms of treatment as appropriate. Adhere to company policy regarding protected health information. Ensure accuracy and timeliness of documentation, including bills and clinical notes. Represent the values and mission of People’s Hospice within the professional and lay community. Perform other duties as assigned by the VP of Operations, Administrator, and Medical Director. Required Qualifications

Registered Nurse licensed as an Advanced Practice Nurse in Florida, accredited by an approved certifying body (American Academy of Nurse Practitioners or American Nurse Credentialing Center). Comprehensive knowledge and well-developed skills in medicine, pharmacology, pain and symptom control, and psychology of loss. Valid Florida Driver’s License and automobile insurance. Understanding and acceptance of People’s Hospice and palliative care principles of care. PECOS certification preferred; DEA preferred. Experience in hospice and palliative medicine preferred. Work Environment

The position requires travel by automobile and exposure to changing weather conditions. Work occurs in varied residential and facility environments based on assigned caseload. Physical Demands

The role requires seeing, talking, or hearing; standing, walking, sitting, and repositioning patients; using hands and fingers to feel objects and reach with hands and arms; and frequently lifting and moving up to 50 lbs. Employment Details
