and willing to travel to practices up to five days a week.

Overview Counterpart Health, a subsidiary of Clover Health, is transforming primary care with its AI‑enabled Counterpart Assistant (CA). CA empowers primary care physicians (PCPs) to deliver better patient outcomes and lower costs through early diagnosis and longitudinal management of chronic conditions.

Responsibilities

Own & Manage Provider Relationships : Serve as the primary point of contact for provider primary care partners, building trusted, consultative relationships to drive CA adoption.

Support Provider Recruitment Activities : Partner with the CA GTM team to execute provider onboarding and training best practices, focusing on in‑office workflows and collaboration with clinical and non‑clinical staff.

Optimize Provider Success & Retention : Implement and oversee a structured engagement plan to ensure providers maximize CA use, achieve measurable success in value‑based care, and remain long‑term partners.

Lead Data‑Driven Decision‑Making : Analyze provider performance metrics, proactively identify trends, opportunities, and risks, and drive interventions to improve provider outcomes and platform utilization.

Influence Best Practices Adoption : Implement CA best practices across various types of PCP practices.

Cross‑Functional Collaboration : Work with internal teams, including sales, product, and operations, to refine Counterpart Health’s offerings based on provider feedback and market trends.

Success in This Role Looks Like

You champion CA with a portfolio of primary care partners, helping PCPs achieve better outcomes in value‑based care.

You demonstrate CA’s value to clinicians and staff, building strong relationships that position CA as a powerful tool improving patient care and clinical outcomes.

Through dedicated support, you enable clinicians and staff to become proficient CA users, embedding CA into their daily workflow.

You consistently achieve key engagement metrics, driving measurable impact for provider practices.

Qualifications

5–7+ years of experience in provider engagement, healthcare account management, or value‑based care consulting, with a strong track record of success in provider adoption and retention.

Proven ability to build collaborative relationships with clinicians and staff, translating operational data into actionable performance metrics and scorecards that improve clinical and financial outcomes.

Deep understanding of primary care and/or ambulatory care workflows, with proven success in implementing, improving, or changing clinical process workflow.

Knowledge of EHRs – ideally in a primary care setting – medical billing, reimbursement models, Medicare Advantage, and risk‑based payment arrangements.

Strong project management skills, prioritizing competing initiatives and driving measurable outcomes.

Willingness to travel up to 75% of the time to support provider practices.

Preferred (but Not Required)

Understanding of value‑based care models, population health, or quality improvement.

Experience implementing value‑based care tools and technology products in primary care.

Knowledge of Medicare insurance, wellness visits, accurate coding, HEDIS, and Stars.
