Position Overview
Providence at Home with Compassus is recruiting a Home-Based Acute Care Occupational Therapist to deliver high-quality, inpatient-level therapy directly into the home setting. The role involves comprehensive evaluations, personalized treatment plans, and discharge planning while working independently and as part of a multidisciplinary care team.
Position Responsibilities
Deliver comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
Evaluate clients’ physical, cognitive, psychological, and perceptual functions to ensure safe and effective function at home.
Develop individualized plans of care in collaboration with patients and families.
Identify and make appropriate referrals related to medical, social, and emotional factors that impact care.
Educate and counsel clients and families on treatment plans, safety concerns, and therapy goals within scope of practice.
Adapt equipment and therapy techniques to the home environment to promote safety and optimal function.
Independently manage a caseload, schedule and coordinate home visits, and collaborate with the interdisciplinary team to ensure cohesive care.
Organize a weekly schedule that balances client visits and participation in required team meetings.
Supervise and instruct Home Health Aides on care plans, including functional skills training and exercise programs.
Provide supervision and case management for clients receiving care from Certified Occupational Therapy Assistants (COTAs), including completing every fifth visit.
Deliver in‑services to staff on new treatments or techniques.
Complete documentation on the same day as visits in line with agency guidelines and reimbursement requirements.
Support the onboarding of new team members by introducing them to clinical practices in the home health setting.
Education & Experience
Preferred: Coursework/Training in Treatment of Adult Hemiplegia and Hand Therapy.
Required: 1‑year experience in acute care, rehabilitation, or home care.
Preferred: Home care experience.
Certifications, Licenses, and Registrations
Required upon hire: California Occupational Therapist License (Vendor Managed).
Required within 30 days of hire: National Provider BLS – American Heart Association (Vendor Managed).
Physical Demands and Work Environment The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.

Compensation Compensation range: $46.57 – $72.29 (base pay range; additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities).
