The inpatient pharmacy at Trinity Health Ann Arbor is open 24 hours a day, 7 days a week, servicing inpatients, the emergency department, operating rooms and multiple procedural areas. Clean room operations are accredited by Joint Commission for adherence to the highest standards of USP 797 and 800.

Position Purpose A Clinical Pharmacist provides medication preparation, distribution services and is prospectively involved with provision of pharmaceutical care and the promotion of high‑quality, cost‑effective, patient‑focused care for populations across the age continuum, from neonatal to geriatric patients. The clinical pharmacist demonstrates pharmacotherapy expertise, participates in interdisciplinary education programs, problem recognition and resolution related to medication usage, actively participates in CQI initiative development and completion, and provides clinical education to pharmacy department staff, students and pharmacy residents.

Qualifications

Education: Bachelor’s degree in Pharmacy; Doctor of Pharmacy preferred

Experience: Prior hospital experience or residency strongly preferred

Licensure / Certification: Registered pharmacist with Michigan license; BLS/ACLS as required; Maintain State Board of Pharmacy continuing education credits every two years

Required Skills: Interpersonal communication, knowledge of medication compounding and dispensing, pathophysiology and pharmacokinetics, analytic organization, basic computer knowledge, commitment to quality improvement, punctuality, customer service orientation, clear written and oral communication, regulatory knowledge, teaching ability

Responsibilities

Interpret and evaluate medication orders to ensure appropriate therapy.

Monitor medication regimens for allergies, interactions, contraindications, and adverse reactions.

Individualize therapy based on renal function, age, weight, labs, and pharmacokinetic parameters.

Manage drug therapy and utilization; communicate with physicians, nurses, and other providers to intervene on unclear or inappropriate orders.

Participate in pharmaceutical care activities: rounds, therapeutic drug monitoring, patient teaching, admission histories, antimicrobial stewardship, pain management, and in‑service education.

Document clinical interventions and participate in quality assurance, drug utilization reviews, pharmacoeconomic analysis, and medication outcome monitoring.

Consult and advise physicians and other healthcare professionals on safe, rational, and cost‑effective medication plans.

Educate providers and staff about quality and cost containment programs related to medication usage.

Direct and monitor pharmacy technicians, interns, and students in medication preparation, packaging, labeling, and distribution.

Direct, monitor, and verify compounding of injectable medications, including epidural infusions, irrigation, large‑volume parenterals, hyperalimentation, and cytotoxic agents, adhering to USP 797/800.

Participate in hospital emergency code responses; prepare and provide emergency medications via carts and kits.

Coordinate pharmaceutical services for specified hospital units and act as liaison between units and pharmacy.

Participate in staff development, continuing education, conferences, newsletters, drug class reviews, and policy updates as allowed.

Precept Doctor of Pharmacy students and pharmacy residents if trained on site.

Perform duties in compliance with all federal, state, and local regulations.

Maintain control and accountability for controlled substances, hazardous medications, and investigational drug regulations.

Maintain legally required records and adhere to programs, ethics, and conduct codes.
