Responsibilities
Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas such as nuclear medicine, oncology, cardiology, etc.
Serve as a subject‑matter expert in pharmacy discipline; provide guidance across pharmacies and to clinical leadership.
Provide specialty pharmacy care services for clinic site(s); function as a key member of the multidisciplinary care team.
Collaborate with physicians, nurses, pharmacists, and other health‑care providers within clinic settings.
Communicate with physicians and other clinicians as needed to facilitate exemplary pharmacy care services to patients.
Demonstrate the value of specialty pharmacy partnership to clinic site practices.
Complete patient education and comprehensive patient assessments, including medication history, lab values, past medical history, physical assessment, and patient interview.
Perform medication therapy and disease‑state management, critically evaluating disease states and adjusting therapy to optimize regimens and improve outcomes.
Create patient profiles and maintain consistent specialty pharmacy prescription throughput processes.
Ensure prescriptions flow through to Ascension Rx specialty pharmacy and are identified by pharmacy staff when appropriate.
Collaboratively address financial issues to ensure patient receipt of needed medications.
Navigate patient prescriptions, including distribution‑limited or out‑of‑network drugs.
Collect and maintain accurate data for analysis and financial reporting.
Apply principles of continuous quality improvement consistent with job expectations.
Review and assess medication orders to identify, prevent, and resolve potential and actual drug‑related problems.
Recommend formulary alternatives when appropriate.
Provide drug information and education to medical staff and patients.
Requirements
Pharmacist licensure obtained prior to hire; willingness to initiate additional licenses within 90 days for states: AL, AR, IA, ID, KS, KY, LA, MD, MI, MS, NE, OK, OR, TN, TX, VA, WV.
Bachelor's or advanced degree in Pharmacy required.
Additional Preferences
Experience in high‑volume clinical pharmacist work, specialty pharmacy, or ambulatory‑care (clinic‑based) settings.
Residency training with PGY2 ideal.
