Registered Nurse (RN) Adult Hospice and Home Health
We are seeking a compassionate and dedicated Registered Nurse (RN) to provide exceptional care to patients in their homes. This role involves coordinating patient care with the interdisciplinary team, families, and referring agencies, while ensuring the highest level of quality care. Key Responsibilities
Patient Care:
Deliver direct nursing care to patients in their homes, following established care plans and physicians' orders. Assessment & Documentation:
Monitor vital signs, document patient conditions, and maintain accurate medical records, including histories, diagnoses, and treatment plans. Medication Management:
Administer and monitor patients' medications, ensuring proper dosage and observing results. Care Coordination:
Collaborate with healthcare team members to assess, plan, implement, and evaluate patient care plans. Patient Support:
Provide personal care, medical attention, and emotional support to patients, ensuring their comfort and well-being. Training & Supervision:
Train and mentor other Registered Nurses, ensuring the effective coordination of care with the broader healthcare team. Family Support:
Communicate patient needs and provide support and guidance to families in the care process. Other Duties:
Perform additional duties as needed to ensure quality patient care. Working Conditions
Work in patients' homes, with potential exposure to various environments and weather conditions. Flexibility in work schedule, with travel required to local patient homes. On-call responsibilities may be required. Reasonable accommodations
may be made for individuals with disabilities to perform essential functions of the role. Qualifications
Education:
Graduate of an accredited Nursing School (Diploma, Associate, or Baccalaureate level). License/Certifications:
Current Texas State Registered Nurse license (or in accordance with Nurse Licensure Compact [NLC] rules). Valid state driver's license.
Experience:
Minimum of 2 years as a Registered Nurse in Clinical, Hospice, or Home Health settings
