LPN Care Coordinator – Skilled Nursing Experience – Spanish Speaking Required
Job Title: LPN Care Coordinator with Skilled Nursing Experience (Fully Remote) (Spanish Speaking Required)
Overview The Care Coordinator is responsible for developing and providing care coordination services directly to Alopex clients’ patients.
Responsibilities
Conduct a detailed review of EMR records.
Develop and maintain individualized Care Plans for patient management.
Implement and coordinate all care management activities across the continuum of care.
Communicate with the patient’s clinical care team in a timely manner.
Monitor patient progress toward desired outcomes through assessment and evaluation.
Maintain accurate and detailed documentation related to medication, problems, goals, interventions, preventative care, etc.
Conduct monthly care calls to enrolled patients.
Address all needs/concerns including any follow-up needed for patients.
Educate patients on chronic conditions and reinforce provider directions related to chronic conditions.
Evaluate and address social determinants of health and link community resources; enter them in the system.
Assess medical conditions and identify and address changes in a clinically appropriate manner.
Adhere to all policies and procedures as outlined by Medicare and Shifox/Alopex.
Perform other duties as assigned.
Qualifications and Skills Required
Bilingual (English/Spanish) – required.
Minimum two (2) years of experience providing patient-centered care.
Minimum one (1) year of experience in skilled nursing.
Experience working with electronic medical records.
Ability to perform nursing assessments, problem identification, and care plan development.
Adaptable to new technology – experience with computer documentation.
Excellent time management skills.
Assessment, evaluation, and problem-solving skills for patient conditions and concerns.
Strong verbal and written communication skills.
Meticulous and dedicated to accurate documentation.
Dependable and focused on achieving goals.
Work Location, Shift & Schedule Remote – full-time. Standard business hours Monday‑Friday, 8:00 am‑5:00 pm or according to client practice business hours. Remote position requirements include reliable internet, a PC or laptop capable of running multiple internet programs, and a quiet professional work environment suitable for patient communication and PHI.
Compensation $26 - $27 / hour.
