Job Description The Case Manager Social Services directs the utilization review of patient charts, treatment plans, and discharge planning pertaining to the quality of care and treatment criteria for patients in a specific department. The role specializes in reviewing information for assigned areas such as Case Management, Geriatrics, Mental & Behavioral Health, and Home Health. The position relies on education, experience, and professional judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. The case manager works under minimal supervision and directs the utilization review of patient charts and treatment plans to ensure quality of care.
Responsibilities
Evaluation and Analysis
Assists in patient admission, transfer, and discharge in a caring and compassionate manner and strives to make the patient’s stay as pleasant as possible.
Conducts intensive patient and caregiver assessment and evaluation to identify needs and recommend immediate and long‑term plans.
Utilizes all equipment, supplies, facilities, and resources efficiently to provide high‑quality health care services.
Partnership and Collaboration
Serves as a liaison between hospital providers, family, patient/member, and physician to resolve complaints efficiently while keeping the patient’s best interests in mind.
Provides suggestions for program development and offers evaluations of existing programs to improve quality and efficiency of health care service delivery.
Manages patient throughput efficiently and serves as an information source for community resources. Assists patients/families by coordinating post‑discharge needs such as ordering DME and setting up home health services.
Coordinates concurrent utilization review and serves as a liaison with payor sources, providing timely updates to ensure appropriate utilization of hospital resources.
Quality
Develops and coordinates high‑quality patient treatment plans based on assessment and evaluation, family dynamics, and individual patient care needs.
Acts as an information and referral source by developing cooperative agreements with community agencies and services to meet patient needs.
Fosters an organizational climate that supports continuous quality improvement efforts and aligns with JCAHO and other regulatory entities.
Observes and adheres to all departmental and hospital policies, safety, quality assurance, and infection control standards.
Promotes the quality and efficiency of performance by remaining current with the latest trends through seminars, conferences, and professional affiliations.
Other Duties as Assigned
Performs other duties as assigned or requested.
Qualifications
Some previous experience in social work
Master’s degree
Current LCSW License
