Caring. Connecting. Growing together. Primary Responsibilities
Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral. Identify the patient’s/family’s physical, psychosocial, and environmental needs and reassess as needed, no less than every 15 days. Initiate and coordinate the plan of care. Document problems, appropriate goals, interventions, and patient/family responses to hospice care. Collaborate with the patient/family, attending physician, and other members of the ID in providing care daily. Instruct and supervise the patient/family in self‑care techniques when appropriate. Maintain accurate and relevant clinical notes regarding the patient’s condition. Required Qualifications
Current and unrestricted RN licensure in the state of practice. 1+ year of clinical experience. Current CPR certification. Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Preferred Qualifications
Able to work independently. Good communication, writing, and organizational skills. Salary & Benefits
The salary for this role will range from $60,200 to $107,400 annually based on full‑time employment. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. All benefits are subject to eligibility requirements. We comply with all minimum wage laws as applicable.
