Hospice Registered Nurse (RN) Clinical Liaison / Admission Nurse – Hospitals & Skilled Nursing Facilities & Clinics (Full-Time)
Location: Walnut Creek, CA (serving surrounding Walnut Creek hospitals & SNFs & Drs. Clinics)
Employment Type: Full-Time Schedule: Wednesday through Sunday, 11:00 AM – 8:00 PM Reports To: Director of Business Development / Director of Nursing Annual Salary Range: $150,000 – $170,000, based on experience and qualifications Position Summary Grace and Glory Hospice & Palliative Care is seeking a Full-Time RN Clinical Liaison based in Walnut Creek, CA, to serve as the primary clinical representative in acute care hospitals, skilled nursing facilities (SNFs) and Drs. Clinics throughout the surrounding Contra Costa County area.
This role combines strong clinical judgment with relationship-driven outreach to support timely hospice referrals, smooth transitions of care, and census growth—particularly for late-day and weekend referrals.
Key Responsibilities Clinical Liaison & Referral Support
Serve as the primary clinical hospice representative in assigned hospitals and SNFs
Identify hospice-appropriate patients through census review, chart audits, and interdisciplinary collaboration
Educate physicians, case managers, social workers, and discharge planners on hospice eligibility, levels of care, and services
Provide compassionate education and support to patients and families
Facilitate timely referrals and smooth transitions from hospital or SNF to hospice care
Admissions & Care Coordination
Conduct bedside clinical evaluations and hospice eligibility assessments
Collaborate with intake, admissions, nursing, and medical directors to expedite admissions
Assist with consents, physician orders, and coordination of initial hospice visits
Participate in GIP, respite, and short-term hospice stay discussions as appropriate
Relationship Development
Build and maintain strong professional relationships with hospital and SNF staff
Attend facility rounds and interdisciplinary meetings
Serve as a trusted clinical resource to referral partners
Represent Grace and Glory Hospice & Palliative Care with professionalism, integrity, and compassion
Documentation & Compliance
Complete accurate and timely documentation related to referrals and evaluations
Ensure compliance with Medicare hospice eligibility guidelines and company policies
Communicate effectively with internal teams to ensure continuity of care
Qualifications Required
Active California RN license in good standing
Minimum 2 years of RN experience (hospital, SNF, hospice, or palliative care)
Strong clinical assessment and communication skills
Ability to work independently across multiple facilities
Valid California driver’s license and reliable transportation
Preferred
Hospice or palliative care experience
Prior clinical liaison, admissions, or case management experience
Familiarity with Medicare hospice eligibility criteria and regulations
