Case Manager, Full Time – Lower Eastside Service Center, Bronx, NY, USA
Job Type: Full‑time
BASIC FUNCTION Provide case management services with a focus on recovery and securing permanent housing to participants.
MAJOR DUTIES
Serve as the primary point of contact for program participants, guiding them through the transition from temporary housing to stable, permanent housing.
Develop individualized service and housing plans with clear goals, timelines, and action steps to address barriers such as income, credit, rental history, and access to supportive services.
Assist participants directly in securing permanent housing, including identifying options, completing applications, building landlord relationships, and preparing for tenancy.
Provide ongoing case management, including support with daily living skills, financial management, employment readiness, and access to healthcare, recovery, and mental health services.
Connect participants with community resources, public benefits, healthcare providers, educational programs, and employment opportunities.
Facilitate problem‑solving and crisis intervention to help participants address challenges that may threaten housing stability.
Conduct monthly home visits and an annual apartment inspection to monitor safety, progress, and overall well‑being, while reinforcing accountability for program participation.
Maintain detailed and timely documentation in compliance with agency policies and funding requirements.
Participate in supervision, case conferences and trainings to ensure high‑quality service delivery.
Perform other duties as assigned.
SCHEDULE Monday–Friday 10:00 AM–6:00 PM
RATE $51,000 – $51,000 per year
LOCATION Bronx, NY 10456

QUALIFICATIONS
Must either have a Bachelor’s degree or 2+ years’ experience providing case management, housing support, or related social services.
Strong understanding of trauma‑informed and recovery‑oriented practices, and knowledge of community resources, public benefits and housing navigation.
Must be organized, self‑motivated, a team player, and computer literate.
Ability to establish rapport and maintain positive working relationships with participants, landlords and community agencies.
