Job Details Overview As a Full‑Time Registered Nurse and Start of Care (SOC) RN with Personal‑Touch Home Care, you'll be integral in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. Utilizing community resources, you'll tailor services to meet the unique needs of individuals and families within their homes and communities.
Responsibilities
Conduct comprehensive assessments of patients and families to determine home care needs.
Provide thorough physical assessments and medical histories.
Continuously reassess patient needs and adjust care plans accordingly.
Collaborate with physicians to develop personalized care plans.
Involve patients and families in the planning process.
Implement preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed.
Offer counseling and health education to patients and families.
Identify discharge planning needs and implement plans prior to discharge.
Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.
Qualifications
Current licensure in the State of Virginia.
CPR certification and a valid driver's license.
Preferably, a minimum of one year of recent acute or long‑term care experience.
Bachelor's degree preferred, with at least one year of experience in home care.
While management experience is not required, responsibility for supervising Home Health Aides and Personal Care Aides.
Strong observation, verbal, and written communication skills.
Excellent problem‑solving abilities and basic math skills.
