Key Responsibilities :
Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
Perform independently under stress and in emergency situations.
Drive between locations, navigating to each location using GPS device or other tools.
Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility, and other personnel involved with patient care.
Assess health status by obtaining relevant health and medical history; perform a physical exam and identify significant health and medical risk factors.
Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs, developing a diagnosis by analyzing and synthesizing collected data.
Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges; prescribe and monitor medications as allowed by prescriptive authority.
Develop a treatment plan in collaboration with the patient and significant others, ensuring quality patient care with multidisciplinary teamwork.
Refer patients to providers as appropriate for consultation or specialized health resources and treatment, ensuring continuity of care.
Supervise nurse practitioners and physician assistants per the collaborative agreement.
Required Qualifications :
Doctor of Medicine or Osteopathy required.
Current unrestricted State license to practice medicine.
Prior supervisor/management experience.
Demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively.
Proven organizational development and project management skills.
Working knowledge of regulations and reimbursement in home health care with proven leadership abilities.
Ability to sit, stand, bend, lift, and move intermittently; lift at least 50 lbs.
Current and valid driver’s license with reliable transportation and valid automobile insurance coverage.
Desired Qualifications :
Board certification in family practice, internal medicine, geriatric, or palliative care preferred.
Preferred 2 years experience in healthcare, with a combination of acute care, geriatric, and/or long‑term care practice.
Experience with geriatric patients preferred.
Location :
Home Office: 770 Kenmoor Ave, Suite 100, Grand Rapids, MI 49546. Main service area: Grand Rapids and surrounding; this position services patients in facilities and private home settings.
Hours :
Office hours: 8:00 AM – 5:00 PM, Monday through Friday.
Pay Range :
$150,000—$200,000 USD.
