Job Summary and Qualifications The Registered Nurse (RN) utilizes the nursing process to direct and deliver comprehensive nursing discharge instructions to patients in discharge suite in a safe and proficient manner according to individual patient needs. Is responsible for preparing discharge documentation, educating patient on discharge instructions, mitigating some discharge barriers for patients discharging home, and answering any discharge questions from patient or family members. This position will assist in the coordination of post‑hospital needs of all patients going home in order to promote quality, continuity of care and safety for the patient. The Discharge RN will meet the needs of the patient, to facilitate timely, and appropriate discharges from the hospital. The Discharge RN will assist in patient advocacy. Provides direct patient care based upon HCA and Las Palmas Del Sol Hospital Standards of Care and Standards of Practice, Polices, Procedures and Practice Guidelines. Provides guidance and supervision to Licensed Vocational Nurses (LVN), Nursing Technicians, Nursing Assistants and Unit Secretaries.

What will you do in this role

Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.

Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post‑hospital documentation.

Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow‑up measures. Translates complex medical terminology to ensure complete knowledge.

Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.

Ensures patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.

Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.

Demonstrates a time‑conscious awareness and ability to prioritize. Distinguishes between low and high‑priority activities.

Licenses and Certifications Required: Current Texas Registered Nursing license or identified compact state license. (If compact license, Texas license must be obtained within 90 days of hire or establishment of Texas residency, whichever occurs first).

Required upon hire (certifications only, within 2 business days of start date):

BLS – required as per St. David’s HealthCare/Las Palmas Del Sol Policy

ALS – required as per St. David’s HealthCare/Las Palmas Del Sol Policy

ENPC – required as per St. David’s HealthCare/Las Palmas Del Sol Policy

TNCC or ATCN – required as per St. David’s HealthCare/Las Palmas Del Sol Policy

National Institutes of Health Stroke Scale (NIHSS) Certification – required within 90 days, after 6 months, within 60 days of hire and annually.

Note: All current employees who had PALS/PLS and were hired prior to 4/1/2022, must obtain ENPC prior to PALS/PLS Expiration.
