Role Description This full-time hybrid Care Coordinator role is based in Virginia, United States, with the flexibility to work from home. The Care Coordinator will serve as a central point of contact for scheduling wound care services, communicating with patients, healthcare providers, and partner agencies to ensure seamless delivery of care. Key responsibilities include managing patient intake processes, facilitating care plans, maintaining accurate records, coordinating schedules for mobile care providers, and supporting the administrative needs of the wound care team.

Qualifications

Strong organizational, time management, and scheduling skills to effectively coordinate care for patients

Excellent communication and interpersonal abilities to liaise with patients, healthcare providers, and partner agencies

Experience with medical record management and maintaining accurate documentation

Proficiency in using healthcare software and tools for scheduling, data entry, and administrative tasks

Problem‑solving skills and the ability to handle patient inquiries with empathy and professionalism

Knowledge or experience in healthcare, patient care coordination, or wound care services is a plus

High school diploma or equivalent; additional education in healthcare administration or a related field is an advantage
