Wraparound Facilitator (QP Facilitator)
At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We’re seeking a full‑time Wraparound Facilitator in the Greensboro, NC area to become an essential part of a caring, community‑focused team.
Your Role in Our Mission As a QP Facilitator on the High‑Fidelity Wraparound team, you will build and lead the Child and Family Team (CFT) to support youth on a mission to stay “at home, in school, and out of trouble.” You will coordinate team activities to achieve this goal while providing direct support and education to families and team members. Using a strengths‑based approach, you will guide the family in accomplishing objectives outlined in the Family Plan and ensure accurate documentation in the UNCG database. You will embody the Wraparound philosophy of “Whatever it takes” to meet the unique needs of youth and families, driving meaningful, lasting success.

Compensation & Benefits
Competitive hourly salary: $43,680 to $52,000 ≈ $21.00 to $25.00 per hour for this non‑exempt position

What We’re Looking For
Education: Bachelor’s degree in Social Work, Psychology, or related Human Services field
Must meet the NC Qualified Professional requirements to include education and experience with the population served
A valid driver’s license, current auto insurance, and a reliable vehicle for work‑related travel
