Behavioral Health Case Manager I - Virginia
Location:
Virtual (Full‑time with required in‑person training sessions as indicated)
Job Summary:
The Case Manager will work with Virginia dual membership members to provide behavioral health and substance abuse support through case management telephonically and/or home visits. The role requires an independent licensure in Virginia and focuses on meeting members in person when contractually required or by member request.
Primary Responsibilities
Use appropriate screening criteria and clinical judgment to assess member needs.
Conduct assessments to identify individual needs and develop care plans to address objectives and goals identified during assessment.
Monitor and evaluate effectiveness of care plans and modify them as needed.
Support member access to appropriate quality and cost‑effective care.
Coordinate with internal and external resources to meet identified member needs and collaborate with providers.
Position Requirements
MA/MS in social work, counseling, or a related behavioral health field, or a nursing degree, with a minimum of 3 years clinical experience in social work counseling covering complex psychiatric and substance abuse or substance abuse disorder treatment; or equivalent education and experience.
Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist to practice within the scope of licensure in applicable states or U.S. territories.
Preferred Qualifications, Skills, and Experience
Experience in case management and telephonic/in‑person coaching with members with complex psychiatric/substance abuse and/or medical disorders.
Experience working with specialty populations.
Prior experience working with the Community Services Board (CSB) and/or Department of Social Services (DSS).
