Nurse Case Manager II – Elevance Health
Join Elevance Health to manage complex and chronic care needs for members as a Nurse Case Manager II.
Base pay range $76,944.00‑$120,912.00 per year
Work Schedule Mon‑Fri 8:00 am‑4:30 pm with 2–3 late evenings a month (11:30 am‑8:00 pm); flexible virtual work with required in‑person training sessions.
Locations Virtual full‑time; must be within reasonable commuting distance of the posting location unless accommodated per law. Eligible states: Illinois, New York.
How You Will Make An Impact
Ensures member access to appropriate services.
Conducts assessments to identify individual needs and develop care management plans.
Implements care plans by facilitating authorizations/referrals.
Coordinates internal and external resources.
Monitors and evaluates effectiveness of care management plans.
Collaborates with Medical Directors and Physician Advisors on treatment plans.
Negotiates reimbursement rates as applicable.
Assists in problem solving with providers, claims, or service issues.
Assists in development of utilization/care management policies and procedures.
Minimum Requirements
BA/BS in a health‑related field and a minimum of 5 years clinical experience, or equivalent combination of education and experience.
Current, unrestricted RN license in applicable state(s); multi‑state licensure required if providing services in multiple states.
Preferred Skills, Capabilities, Experiences
Certification as a Case Manager.
