The health Home care manager will help children that have a medical or mental health diagnosis stay safe in their home and communities, by connecting them with the right providers and specialists and coordinate with all care team members involved to work collaboratively to ensure to best outcome for member.
Tasks and responsibilities Care manager will conduct assessments to identify member’s and family’s needs. CM will then create a “plan of care” that will include the needs identified In the assessments and will add appropriate goals and tasks to address the needs. He CM will then provide services to help member achieve those goals.
Duties include but not limited to:
Housing
Physical and mental health
Education
Food security
Activities of daily living
Task and responsibilities
Conduct assessments timely
Coordinate services
Timely documentation of progress
Meet with Member and family monthly
Being in touch with care team members consistently and as needed. Including Physical, Mental and Dental providers, MCO, School teacher, principal.
Keep members profile in compliance at all times, by ensuring that all assessments, Progress notes and other member documentation and details are up to date all the time.
