Service Areas: Tarpon Springs, Palm Harbor, Safety Harbor, Oldsmar, Clearwater, Dunedin, Largo, Pinellas Park / Gandy area.

Overview Pinnacle Home Care Florida’s largest Medicare‑certified home health provider has been delivering high‑quality patient‑centered care for over two decades. We are looking for Licensed Practical Nurses to join our award‑winning team.

Key Responsibilities

Administer skilled nursing care under RN supervision.

Perform curative, rehabilitative or preventive care tasks as outlined in patient care plans.

Utilize clinical expertise and nursing education to make informed independent decisions that support patient care and promote positive outcomes.

Complete visit reports promptly and accurately within the electronic medical record system to support high‑quality care and regulatory compliance including all required documentation.

Confirm daily visit schedules to ensure seamless coordination with other staff.

Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.

Understand and adhere to the agency’s process for managing and addressing customer complaints.

Maintain acute awareness of patient rights ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.

Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.

Qualifications

Must have a current LPN or LVN license (state‑specific).

Graduate of an accredited nursing program.

Valid BLS certification.

Minimum one year of nursing experience (home health or medical/surgical preferred).

Excellent communication, organizational and time management skills.

Must have a valid driver’s license and access to a reliable and insured vehicle.

Ability to visit homes that may not be wheelchair accessible.

Sufficient hearing, eyesight and dexterity to perform physical assessments and demonstrate care techniques.

Engage in physical activities such as walking, standing, stooping and occasional lifting or turning of patients.
