Overview Soleo Health is a national provider of specialty pharmacy and infusion services administered in-home or at alternate care sites. As an Infusion Nurse you’ll deliver primary patient care, including assessments, care planning, evaluation, and education for patients receiving infusion therapy or other nursing services, according to our care matrices and best practices.

Responsibilities
Provide direct care for all assigned patients, initiating and maintaining communication with care team members, pharmacists, prescribers, dietitians, home‑patient representatives, and community resources.
Evaluate and educate patients/care partners to safely administer home infusion therapy and provide additional instruction as needed.
Assess home environments and identify interventions required for safe care delivery.
Collaborate on care plans with team members and evaluate response to therapy during each encounter.
Maintain caseload scheduling via the approved company platform and communicate any changes to appropriate team members.
Coordinate care through the company platform.
Participate in the nurse on‑call rotation to ensure continuous nursing support for customers.
Document all communications at the time of service per Soleo policy.
Maintain strong knowledge of therapies provided by Soleo.
Ensure required patient documents are completed and returned to the branch office within the acceptable timeframe.
Submit plans of treatment timely, in compliance with state guidelines.
Schedule
Per diem schedule, 20‑24 hours per week, variable schedule including evenings and weekends as needed.
Knowledge of IV access devices (IV lines, ports, and infusion medication administration).
Ability to drive up to 1 hour to see patients as needed; mileage reimbursement and hourly rate of pay provided.
Licensed RN in the state of Michigan.
Requirements
RN degree; BSN preferred.
Advanced certification in specialty areas (CRNI, ONS, IgRN) preferred but not required.
On‑call participation, nights and weekends, providing as‑needed support outside regular hours.
Administrative oversight responsibility overseeing workflow, compliance, and resource management.
Emergency availability for prompt response during non‑standard hours, including nights, weekends, and holidays.
Minimum 3 years of nursing experience; infusion therapy background preferred.
Valid nursing license for the applicable home state.
ICU and/or cardiac care experience preferred.
Proficiency with Microsoft Outlook and Office Suite.
Valid state‑issued driver’s license with no significant restrictions.
