Requirements
Doctorate in Physical Therapy or Bachelor’s in Physical Therapy with NPTE license
Graduate from an accredited school
Minimum one year of experience
Current state license as a Registered Physical Therapist / PT or in process
Current CPR certification
Additional client facility requirements to be determined
Duties & Responsibilities The Physical Therapist/PT is responsible for assessing referred patients requiring rehabilitation services. They will provide direct patient care, assess medical condition, functional capabilities, and potential for rehabilitation. The PT will establish and administer a treatment program, set realistic goals, document patient condition, and educate patients and families.
Develops a plan of care for each physical therapy patient.
Provides skilled physical therapy services/interventions per physician orders.
Assesses patient needs, plans for, evaluates and modifies care to meet goals of interventions.
Collaborates with all disciplines to plan and evaluate team goals for each patient.
