Wellness Nurse (LPN) – Full Time | Flexible Shifts | $32.00–$38.50/hr | Worcester, MA
The Elms Assisted Living at The Briarwood Community
is seeking a compassionate and skilled
Wellness Nurse (LPN)
to join our care team. This full-time role offers flexible scheduling and a competitive hourly rate of
$32.00–$38.50 , providing an excellent opportunity for an LPN committed to delivering high-quality, resident-centered care. Every other Weekend required.
Key Responsibilities
Provide nursing care and support to residents in accordance with individualized care plans.
Administer medications and treatments following physician orders and facility protocols.
Monitor resident health status, report changes promptly and assist with assessments.
Collaborate with RNs, CNAs, physicians, therapists, and families to ensure coordinated care.
Maintain accurate documentation, medication records, and progress notes.
Support wellness initiatives, education, and preventive care practices within the community.
Qualifications
Current Massachusetts Licensed Practical Nurse (LPN) license in good standing.
Strong clinical, organizational, and communication skills.
Ability to work effectively with residents, families, and interdisciplinary team members.
Experience in assisted living, long-term care, wellness programs, or skilled nursing preferred.
Commitment to providing compassionate, person-centered care.
