Position Summary Under the direction of the Medical Director (or designee) provides patient services* including obtaining medical histories and conducting physical examinations on a primarily outpatient basis, assisting in diagnosing and evaluating patients with bleeding and clotting disorders, and prescribing treatments and medications related to bleeding and clotting disorders under the supervision of hematologists associated with the clinic. This position has a mix of outpatient clinic evaluation and follow-up and day hospital coverage on an acute basis. On occasion, cross coverage of inpatient consultative hematology service may be required. The provider also coordinates activities of the clinic and the professional treatment team, assists in coordination of research, and provides education regarding bleeding and clotting disorders to patients, their families, health care practitioners and the public. This position is expected to perform most responsibilities independent of direct supervision. *Scope/limitations of patient services and professional practice, including delegated "medical" acts and prescribing, as well as level of physician supervision for Registered Nurses and Advanced Practice Nurse Prescribers are defined by state law in the Wisconsin Department of Regulation and Licensing Administrative Code, Chapter N6 and 8 and for Physician Assistants in Chapter Med 8.

Responsibilities
Provides and documents medical care to patients with bleeding and clotting disorders, primarily those with hemophilia, severe von Willebrand Disease, and rare coagulation disorders, by implementing the treatment plan developed by the professional treatment team, and acting as a consultant to hospital inpatient and outpatient medical providers while patients are being cared for at community hospitals and outreach clinics.
Provides timely management of surgical and other medical procedures for bleeding and clotting disorders, including: coordination with Therapeutic Products, the Home Care Program, the preop clinic and all other providers; acting as a consultant to hospital inpatient and outpatient medical/surgical providers; and providing education and support to patients and families while they receive care at community hospitals.
Coordinates management of patient’s bleeding and clotting disorders prior to and following invasive procedures and surgery, implementing the treatment plan developed by the professional treatment team, assists in the maintenance of the CCBD home therapy program, monitoring patient compliance, including the periodic review of protocols and procedures.
Collaborates with the Department of Benign Hematology at Froedtert Hospital to coordinate care for adult patients with bleeding and clotting disorders.
Takes medical histories and provides complete medical examinations, assists in diagnosing and prescribes treatments and medications for patients with bleeding and clotting disorders under the supervision of the CCBD Medical Director or other CCBD-associated hematologists. Based on evaluation, formulates and documents a plan of care in conjunction with the collaborating physician. Evaluates outcomes and revises a plan of care in conjunction with protocols or collaborating physician. Develops problem lists with associated differential diagnoses.
Evaluates and initiates preventive, screening, diagnostic and therapeutic measures in accordance with approved protocols or physician consultation. Implements a plan of care. Performs procedures in accordance with approved protocols or physician collaboration. Initiates appropriate referrals
Provides patient and family education, anticipatory guidance and counseling. Functions as a primary educator of patients and families with bleeding and clotting disorders by providing information regarding pathophysiology and treatment and responding to patients’ and families’ questions and concerns.
Coordinates discharge of in-patients on anticoagulation therapy in collaboration with primary medical team and nursing staff. Includes identifying designated provider responsible for following levels and dose adjustments, identifying appropriate lab drawing facility and facilitates process of obtaining home monitoring device when appropriate. Make arrangements for clinic follow-up and schedules imaging as needed prior to discharge. Track anticoagulation consultations; summarize annually.
Functions as part of a comprehensive team, provides cross coverage for other team members, and provides assistance with process development, implementation of new programs and other initiatives, and other projects/duties as assigned. Provides assistance to Nurse Coordinators, Pharmacist and Manager, CCBD and other staff when applicable and to support the distribution and reimbursement of factor replacement products.
Possesses and demonstrates outstanding leadership skills to be able to mentor and lead staff as well as motivate disparate groups around quality improvement initiatives. Demonstrates the ability to work proactively and collaboratively with people at all levels within our organization.
Acts as a liaison between the patient, their family, the physicians and other members of the treatment center, as well as acting as a resource to the patient’s primary care physician and other health care providers involved in the patient’s care.
Coordinates the hemophilia and /or coagulation clinic and services provided by the treatment center team members including: the scheduling/organizing of the Comprehensive and Diagnostic/Coagulation Clinics, scheduling of patient appointments, and reviewing of outgoing correspondence and treatment recommendations.
Obtains Collaborative Institutional Training Initiative (CITI) training certification to include Good Clinical Practice which will enable participation of enrolling our patients into research studies for which they qualify. Assists as needed in the coordination of research and prevention activities involving the patient population, including coordinating laboratory studies and data compilation/maintenance, as well as educating patients and families regarding the purpose of the research. Complete any essential research trial-specific training, assist with clinical trials in thrombosis and hemostasis and perform research assessments as needed.
Maintains current knowledge base and skills through participation in continuing education, professional meetings and conferences, and reviewing current literature.
Performs other duties as required or assigned which are reasonably within the scope of the duties in this job classification

Qualifications Education
Master's Degree in nursing or master's degree in physician assistant studies (appropriate education and/or experience may be substituted on equivalent basis) required
Experience
1-3 years clinical experience required
Experience in triaging, evaluating and treating basic medical conditions required
Knowledge, Skills and Abilities
Excellent organizational ability with attention to detail required
Ability to work independently under minimum supervision as well as to function as a productive team member required
Excellent verbal and written communication and teaching skills required
Strong interpersonal skills required
Working knowledge of computer databases and word processing applications required
Venipuncture skills required
Demonstrates strong interpersonal and teamwork skills and has ability to work with diverse constituencies throughout Versiti and partnering organizations required
Valid Wisconsin driver’s license required
Licenses and Certifications
