Health & Housing Navigator

A community organization based in Portland seeks a Health Related Social Needs Coordinator to address social determinants of health related to housing instability and homelessness. The coordinator will develop support plans, coordinate referrals, and provide housing navigation assistance. Preferred candidates should have strong communication and organizational skills, as well as a background in social work.
