LTSS Service Coordinator – Case Manager (Bilingual in Spanish/English)
Elevance Health is seeking an LTSS Service Coordinator, also known as a Case Manager, to conduct service coordination functions for a defined caseload of individuals in specialized programs.
Location & Schedule Primary field‑based position. Candidate must reside in Miami‑Dade, Broward, Lee, Hillsborough, or Palm Beach County. Schedule: Monday‑Friday, 8 a.m.‑5 p.m.
Responsibilities
Perform face‑to‑face program assessments using predefined tools; apply motivational interviewing techniques to evaluate and manage individuals’ waivers (LTSS/IDD) and BH or PH needs.
Identify members with potential clinical health‑care needs and coordinate care with the clinical healthcare management and interdisciplinary team.
Manage non‑clinical needs of members with chronic illnesses or disabilities to ensure cost‑effective utilization of long‑term services.
Document short and long‑term service and support goals in collaboration with the member’s care team.
Identify members who may benefit from alternative service levels or other waiver programs.
Act as mentor or preceptor for new staff and support process improvement initiatives.
Submit utilization/authorization requests to utilization management with supporting documentation.
Report critical incidents to state and county agencies, including Adult Protective Services and Law Enforcement.
Assist with appeals, grievances, and state audits.
Minimum Requirements
B.A./B.S. degree and at least 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
Specific education, years, and type of experience may be required based on state law and contract requirements.
Preferred Qualifications
B.A./B.S. degree in a health‑care related field.
Bilingual in English and Spanish.
Experience with LTSS, case management, social work, or hospital discharge planning.
