Cold Hollow Family Practice is a busy, established primary care practice, part of Primary Care Health Partners, Vermont's largest independent primary care group. Responsibilities
Provide direct support and care to patients in a primary care setting
Conduct patient assessments and develop individualized treatment plans
Implement therapeutic interventions, including group therapy and cognitive behavioral therapy
Assist with crisis intervention and de-escalation techniques
Collaborate with the interdisciplinary team to coordinate patient care and case management
Monitor and document patient progress, behaviors, and responses to treatment
Provide education and support to patients and their families
Requirements
Master's degree in psychology, social work, or mental health counseling field (licensed or license-eligible in the State of Vermont)
Experience working in a behavioral health or mental health setting
Knowledge of gender issues counseling and sensitivity to diverse populations
Familiarity with patient assessment tools and research methodologies
Strong communication and interpersonal skills
Ability to work collaboratively in a team environment
Excellent organizational and time management skills
