We offer competitive pay, flexible scheduling, a 401(k) with employer match, and ongoing professional development—so you can grow your career while changing lives every day.**How will you make an impact & Requirements****Millennium Home Care (an affiliate of Millennium Healthcare, LLC)****PRN Speech-Language Pathologist (SLP)****Make a Difference in Home Health****Responsibilities:*** **Patient Assessment:** Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.* **Treatment Planning:** Develop individualized treatment plans that address specific patient goals, including speech sound production, language development, fluency, voice disorders, and swallowing difficulties.* **Therapeutic Interventions:** Provide direct therapy services, such as articulation therapy, language therapy, fluency therapy, voice therapy, and swallowing therapy, using evidence-based practices.* **Patient Education:** Educate patients and their families on strategies to improve communication skills, swallowing safety, and overall quality of life.* **Collaboration:** Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to develop comprehensive care plans and ensure optimal patient outcomes.* **Documentation:** Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.* **Compliance:** Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.**Requirements:*** **Licensure:** Current and valid Speech-Language Pathologist (SLP) license in the state of Florida.* **Experience:** Minimum of 1 year of experience in hospital, rehab, or skilled nursing facility is required.* **Skills:** Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.* **Professionalism:** Strong work ethic, reliability, and a commitment to providing high-quality care.* **Transportation:** Reliable transportation to visit patients in their homes.**Benefits:*** **Competitive Pay*** **Flexible Scheduling*** **401(k) Plan with Employer Match*** **Professional Development Opportunities*** **Supportive Work Environment****Join our team and make a lasting impact on the lives of our patients.**locations:
