The CATCH Team are part of the Integrated Specialist Nursing team, comprising the CATCH Team (Community Assessment and Treatment of Children at Home), Long Term Conditions, Training, Home Respite, and the Nurse‑led Continence Service.

We work Monday to Friday until September 2026, providing services from 8am-9pm, 7 days a week. The post holder will assess acutely unwell children in their own homes/community, taking referrals from GPs, EMAS, and the acute hospital, with the aim to reduce hospital admissions and manage the child safely within the community setting.

Main duties The post holder will work as part of a team whilst working autonomously in GP hubs and community settings as first contact, diagnosing and managing children under 6 experiencing acute illness, prescribing as required, and providing an alternative to hospital admission. They will also educate and empower parents for safe and effective home care.

Additionally, the post holder will participate in a telephone on‑call rota, cross‑work with the wider Integrated Specialist Nursing Team, and take part in End of Life Care while being supported by the wider team.

Person Specification We are looking for a forward‑thinking, dynamic nurse who enjoys innovative, high‑standard practice, embraces change and remains flexible.

Interview criteria

Paediatric nursing qualification

NMP

Evidence of paediatric assessment course

Experience of community based employment

Palliative care or end of life experience

Evidence of working with acute illness

Evidence of working with long term conditions

Competency in IVABs

Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

Salary: £38,682 to £46,580 a year (on‑call pro rata, when reinstated in September 2026).
