Position Overview Our Hospice Coordinator is responsible for the efficient and effective facilitation of referrals to the post‑acute home‑care/hospice agency once identified in the acute care setting; the provision of education to acute care staff, including physicians, regarding services available from home‑care/hospice; and may also be involved in business development activities as they relate to business expansion or new program development.

What Will You Do

Maintain daily contact with case managers, discharge planners, and units to offer assistance and ensure patients referred for services are followed up on in a timely and efficient manner.

Interact with healthcare team members to answer any questions related to home‑care/hospice and community‑based services as they plan for patient discharges from the acute care setting.

Coordinate care for clients going home from the hospital who have chosen the agency’s services.

Review and gather information from the hospital medical record, copying portions for transmission as needed.

Document initial assessment of home‑care needs and potential plan for services in compliance with agency policy.

Arrange services in accordance with established admission criteria, client care needs, and client preferences.

Obtain physician orders for initiation of home‑care/hospice services in accordance with agency policy/procedure and third‑party regulations.

Utilize interview, observation, and evaluation in assessing clients and apply clinical judgment, consistent with practice standards, in determining appropriate home‑care services and making recommendations to the physician and client.

Interact with healthcare team members, clients, and their families to answer additional questions regarding home‑care/hospice services being requested; provide clients and families with written and/or verbal information as appropriate.

Make appropriate referrals for evaluation/care to other services as needed, and coordinate care with others to ensure effective and efficient care is provided.

Minimum Qualifications

Bachelor’s degree or equivalent level of experience.

Excellent communication and customer service skills.

1‑2 years of sales experience, preferably in hospice.

Displays optimal critical thinking skills.

Knowledge of Medicare rules and regulations.

Working knowledge of managed care environment.

Experience multi‑tasking on multiple computer systems.

Must have a valid driver’s license and reliable transportation to and from work site.

Benefits Highlights

Graduate of an approved clinical education program and current clinical license in the state of employment.

Preferred: minimum of two (2) years’ experience in acute care setting, customer service, sales/marketing role or relevant experience.
