Job Summary Under limited supervision, the Clinical Pharmacist provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and health care providers to deliver high‑quality care. Responsibilities include reviewing prescriptions for safety and appropriateness, dispensing or compounding medication, interviewing patients for medication history and counseling, teaching and educating patients, collaborating with healthcare providers for coordinated patient education, supervising support personnel, maintaining medication inventory and records, ensuring compliance with regulatory procedures, conducting patient care area inspections, and providing services such as therapeutic drug monitoring, anti‑coagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service, and adverse drug reaction reporting.
Qualifications
Bachelor of Science in Pharmacy or Doctorate of Pharmacy from an accredited College of Pharmacy, or completion of a foreign equivalency examination.
Registered Pharmacist and Controlled Substance licensure in the State of Michigan obtained within 3 months of entry into the job.
