Position Summary We have an exciting opportunity to join our team as a Per Diem Home Care Intake Nurse - Days.
Must have 1 year of RN Experience.
In this role, the successful candidate completes a comprehensive assessment of patients referred to the Agency for Home care services from hospitals, skilled nursing facilities, rehab facilities, and from the community. Responsible for the planning to admit the patient to the Agency. Determines and ensures client eligibility and authorization to receive requested services and notifies all pertinent staff of approval or denial of requests.
Job Responsibilities
Performs other duties as assigned.
Establishes and completes a realistic assessment and discharge plan based on patient/family needs and abilities.
Takes responsibility for obtaining accurate, pertinent information from health care records (hospital, skilled nursing facilities, rehab facilities and primary physicians) patient caregiver to create a smooth transition and individualized plan of care in home environment. Assesses for improved safety by ordering DME as needed, providing private hire lists for assistance in ADL functions and explains third party payor reimbursement for home health services.
Collaborates with multidisciplinary team to discuss issues impeding optimal functioning and/or smooth transition to home. Liaison for clinicians with managed care organization to acquire authorization for services needed.
Identifies and assesses for potential or actual safety risks, hazards, barriers that block ability for optimal functioning.
Complete and provide accurate, complete and timely documentation reflecting intake coordination for patients referred to the Agency from hospitals, skilled nursing facilities, rehab facilities and the community based on Federal, Department of Health and The Joint Commission requirements. Ensures timely flow of information to optimize operations (for example: referrals, ordering equipment, interviews, etc.)
Assists agency leadership in achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participates in quality assessment and performance improvement initiatives.
Demonstrates caring behaviors towards patients, caregivers, co-workers and other health care team members.
Minimum Qualifications To qualify you must have a Diploma OR Associates Degree in Nursing. Current and Valid New York State Registered Professional Nurse license. 1-2 years Acute Care Facility or Home Health, Hospice and/or Long Term Home Health Program experience. Excellent communication (both oral and written), interpersonal, customer service skills. Critical thinking skills. Working knowledge of computers.
Preferred Qualifications Bachelor's Degree in Nursing.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $52.00 - $52.00 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
