Responsibilities

Work as a member of a team of professionals who provide outreach, engagement, case management, and service coordination services to adults who are homeless and have a mental illness or co-occurring mental illness and substance use disorder.

Identify eligible individuals and assess their needs.

Plan and link individuals to resources.

Monitor and evaluate services and supports.

Participate in street outreach, engaging people who are homeless at shelters or other community locations.

Qualifications

Ability to participate in street outreach and engage people who are homeless, possibly living outdoors.

Active Maine driver’s license and reliable transportation.
