We are currently seeking a compassionate, team‑oriented Care Team Nurse (LPN/RN) to support patients across two clinic locations.
YOUR ROLE Provide practice‑management support to the clinical team, embracing the Patient‑Centered Primary Care Home (PCPCH) by recognizing the patient as the central focus. The Care Team Nurse, in collaboration with other health‑care professionals, addresses patients' wellness, acute illness, chronic disease, disability, and end‑of‑life needs. Practice includes telephone advice for minor acute problems; patient education on self‑care; ongoing management of stable chronic disease; monitoring of acutely ill patients during routine office visits; and performance of procedures within the scope of professional nursing practice. Under PCP direction, provides patient education on self‑care, including medication administration, nebulizer use, inhaler technique, diabetes management, wound care, and self‑injection. Explains treatments and procedures to patients.
Assists providers with patient examinations and procedures (e.g., minor surgery, casting, nebulizer treatments, spirometry, EKG) as well as medication administration, based on clinical needs and scope of nursing practice.
Accurately observes, analyzes, and interprets data both verbal and written, communicating clinical findings. Recognizes abnormal and critical findings and takes appropriate action, including notifying the provider.
