Job Overview Position: Social Services Designee SSD (Social Services Designee)

Employer: Midwest Health, Inc.

Location: Worksite attendance required.

Responsibilities

Assist in implementing and directing social service programs in accordance with federal, state, and local standards.

Ensure residents' physical, mental, emotional, and social needs are met on an individual basis.

Comply with facility personnel, safety, and corporate policies and procedures.

Report allegations of resident abuse/neglect immediately to the Administrator or Nurse Manager on Call.

Treat residents, family members, visitors, and team members with respect and dignity.

Maintain confidentiality of all resident and family information.

Assist the Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.

Meet with administration, medical, and nursing staff to plan social services.

Coordinate admissions, readmissions, room moves, and patient inquiries.

Participate in discharge planning and development of social care plans.

Interview residents/families privately and involve them in planning social service programs.

Provide information regarding Medicare/Medicaid and other financial assistance programs.

Complete necessary forms, reports, and submit to the Director.

Coordinate resident Medicare/Medicaid benefits and VA/insurance benefits.

Maintain social service progress notes.

Coordinate social service activities with other departments.

Make routine visits to residents and provide services.

Attend in-service training and safety drills.

Report incidents/accidents immediately.

Assist with evacuations during emergencies.

Qualifications

Bachelor’s degree in social work or related field preferred.

Social service designee certificate or license required.

Basic computer skills.

Proficient in English (reading, writing, speaking).

Ability to work beyond normal hours, weekends, and holidays as needed.

Basic driving policy compliance if required.

Physical Requirements

Regular worksite attendance.

Must be able to lift up to 10 lbs occasionally; lifting 11–25 lbs not required.

Carry up to 10 lbs occasionally; carrying 11–25 lbs not required.

Reach above shoulder height occasionally; reach at shoulder height frequently; reach below shoulder height frequently.

Push/Pull occasionally.

Hand manipulation: grasping frequently, handling constantly, fingering occasionally.

Capacity to twist, bend, crawl, squat, kneel, crouch, climb, and balance.

Must be able to use a transfer/gait belt or obtain assistance when lifting objects over 25 lbs.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and hostile patients.

Must be able to see, hear, and use prosthetics as needed.
