Per Diem Community Support Counselor (CHAMP)

Per Diem Community Support Counselor (CHAMP) Location: Morris Plains, New Jersey

Employment Type: Per Diem

Pay: $19.50 per hour

Shift: Monday through Sunday, 20 hours

Responsibilities

Responsible for the implementation of IRPs, performing IMR services, and providing continuous input to all changes of consumer IRPs

Provides on-site case management services to assigned caseload and supervision to consumers

Facilitates a variety of social, recreational, and therapeutic groups, encourages consumers to participate

Utilizes crisis intervention skills as needed and reports all consumer concerns to program managers

Must be able to follow agency’s documentation procedures correctly when updating consumers’ electronic charts

Qualifications

BSW in social work, BS/BA in psychology, or a related field

Valid Driver’s License
