Responsibilities
Conduct outreach and engagement activities to identify and connect eligible individuals with programs such as Substance Abuse Treatment, PRP and MTS services.
Build and maintain positive relationships with clients, families, community partners, and referral sources.
Assist in client intakes, ensuring completion of required documentation and coordination of services.
Provide follow‑up support to ensure clients remain actively engaged in treatment and rehabilitation programs.
Collaborate with counselors, case managers, and clinical staff to develop and implement individualized service plans.
Participate in community events and agency outreach initiatives to promote program awareness.
Maintain accurate and timely records of outreach activities, client contacts, and service outcomes in compliance with agency and COMAR standards.
Identify barriers to service participation and work with the team to develop strategies that support client access and retention.
Support clients in connecting with community resources such as housing, employment, education, and health services.
Attend team meetings, training, and supervision sessions to enhance professional growth and program effectiveness.
