Social Worker – Erickson Senior Living Lantern Hill by Erickson Senior Living – a 20‑acre, continuing care retirement community in New Providence, New Jersey – is looking for a resident‑centered Social Worker to join our skilled nursing, long‑term rehab, assisted living, and memory care teams. The role focuses on providing high‑quality social work services to residents and their families.

Compensation: $70,000 – $80,000, commensurate with experience.

How You Will Make an Impact

Complete psychosocial assessments, MDS, and other required documentation per policy and regulatory standards.

Coordinate the Social Work function from admission through discharge, including room changes and transition planning.

Participate in the care plan process by collaborating with interdisciplinary teams and families to develop resident‑centered plans and prescribe psychosocial interventions.

Provide bereavement support to residents, families, and staff as appropriate.

Monitor and document psychosocial and medically related social service needs routinely.

Maintain working knowledge and ensure compliance with federal, state, and local regulations and professional standards.

Investigate and coordinate resolution of grievances and customer service issues, collaborating with residents to facilitate decision‑making at the most appropriate level.

What You Will Need

Social Worker degree required; MSW preferred.

Minimum of 3 years of experience working with older adults in post‑acute/long‑term care, with knowledge of geriatrics and dementia.

Proficiency with Microsoft Office (Word, Outlook, etc.).
