JOB RESPONSIBILITIES
Referral Management -
Track data on number of referrals, referral types and monthly available visit types for purpose of managing supply and demand. Coordinate and maintain statistical data on number of calls and disposition and participates in Performance Improvement process for the department. Ability to gather clinical, medical, and demographic data to determine appropriate referral and level of care. Document information and refer patients to internal/external services as appropriate within timely manner.
Customer Service -
Responds promptly to intake requests. Initiates referral process in a manner to be regularly and conveniently available to meet the needs of CHMC's patients as determined by department management and the medical staff.
External Evaluation -
Triage and evaluate referrals. Assess for appropriateness and/or determine options within or outside of CCHMC. Triage and evaluate the patient need vs. service availability of patients and families for outpatient referrals. Advocates for the needs of the child and adolescent patient and family within the hospital and the community.
Patient Assessment -
Assess patient need to determine phone call urgency. If routine, assess needs and provide specific referral options. If urgent, transfer to appropriate clinician.
Customer Service -
Develops skills and knowledge to interact with third-party payers and managed care. Maintain professional relationship with referral sources and managed care contacts. Authorize outpatient care and collect specific benefit information.
JOB QUALIFICATIONS
Bachelor's degree in a related field
2 years of work experience in a related job discipline.
