The Telephonic Care Coordinator is responsible for facilitating, promoting and advocating for patients’ ongoing self-sufficiency and independence. This position focuses on assessment and planning for an identified group of patients (Community Well/Emerging High‑Risk Population), managing their care in homes or community settings through telephonic health assessments and coordination, with focus on disease management and patient independence.

Primary Responsibilities

Engage members and families via telephone to coordinate services, community resources, and treatment needs.

Deliver a holistic, person‑centered approach to coordinated care based on member needs.

Identify early risk factors and conduct ongoing assessments, documenting in an electronic file.

Create, review, and revise care plans with a holistic focus.

Advocate for the member and collaborate with the member’s PCP to deliver necessary services.

Build relationships with members and families, assisting them with health choice coordination.

Document cases in a timely manner and coordinate with state agencies, community resources, and providers.

Act as the primary point of contact for members and the Interdisciplinary Care Team (ICT).

Required Qualifications

Bachelor’s Degree in Social Work, Public Health, or a related field.

2+ years recent experience in case management, home care, long‑term care, or acute/rehab settings.

1+ year experience directly working with individuals with complex medical or behavioral needs.

Solid computer proficiency (Microsoft Word, Outlook, Internet).

Driver’s license and access to reliable transportation.

Reside within the state of VA.

Preferred Qualifications

Experience with Medicare and/or Medicaid.

Experience with complex populations.

Telephonic / call‑center experience.

Managed care / case management experience.

Pay is based on several factors, including local labor markets, education, work experience, certifications, etc. The hourly pay for this role ranges from $23.89 to $42.69 per hour for full‑time employment. We comply with all minimum wage laws as applicable.
