Job Description
Will be working remote when not in the field. 80% field, 20% remote.
Manages 60-80 patients per caseload.
Will be given a lap tap, cell phone and office supplies.
Will be traveling to member homes, long term care facilities, physician offices, hospitals and other provider offices.
Qualifications
We are looking for field case managers who have experience with geriatric populations.
Must be willing to travel to patient’s homes.
Must be computer literate. Big! – Will be asked if they can operate Windows based systems.
701B Comprehensive Assessment Certificate is strongly preferred – an exam you take that allows you to conduct 701B type of assessment.
IT will provide candidates with the skills and knowledge they need to succeed as a case manager or assessor working with elders and adults with disabilities.
