UT Physicians Pediatrics High Risk (TMC) clinic . You will provide professional social work services addressing medical, psychiatric and social needs of children with complex conditions and illnesses, using a biopsychosocial perspective.

Location UT Physicians Pediatrics High Risk (TMC), 6410 Fannin St., Houston, TX 77030. Full‑time schedule: Monday–Friday, 8 am–5 pm.
Key Accountabilities
Conduct comprehensive assessments of patients’ social, emotional, environmental and financial needs.
Plan treatment courses or refer cases to appropriate professionals or agencies.
Gather data from patients and families to inform essential teaching enterprises and social services.
Collaborate with physicians and the multidisciplinary team to coordinate care, enhance patient experience and achieve goals. Develop patient goals, monitor progress, address barriers and evaluate outcomes.
Provide individual and group counseling.
Prepare detailed reports and may videotape sessions for review.
Participate in teaching, staff development and serve as instructor in the social work graduate student practicum program.
Educate physicians, nurses and other providers on community resources and psychosocial impacts on care needs.
Solicit assistance from religious, social and welfare agencies in patient treatment. Interact with social work staff and psychiatric residents.
Use crisis intervention, conflict resolution and mediation to address patient/family concerns.
Respond to emergency situations.
Perform other duties as assigned.
Certification / Skills
Knowledge of DSM‑5 criteria.
Licensed Master Social Worker (LMSW) by State of Texas required OR
Licensed Clinical Social Worker (LCSW) by State of Texas required.
Minimum Education Master’s Degree in Social Work from a school approved by the Council on Social Work.
Minimum Experience Four years of social work experience, with at least one year of evidence‑based, client‑centered counseling (group or independent) required.
Physical Requirements
Exerts up to 50 pounds occasionally, up to 20 pounds frequently, or up to 10 pounds constantly to move objects.
Residency Requirement Employees must permanently reside and work in the State of Texas.
