The

Children's Long-Term Support (CLTS) Waiver Program

is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings. As a Care Coordinator, you'll be the primary point of contact for families, helping them access and navigate essential services.

What You'll Do

Assess children's functional abilities using approved tools Develop and implement individualized service plans with families and providers Facilitate team meetings and coordinate services based on family-centered goals Maintain accurate documentation and meet all regulatory timelines Collaborate with internal teams and external agencies to support families ️ Testify in legal proceedings when required Communicate clearly with families, providers, and team members Participate in staff development, training, and supervision Perks & Benefits

Qualifications

Bachelor's degree in a human services field (e.g., Social Work, Psychology, Special Education, Counseling, etc.) Minimum 1 year of experience working with children with disabilities Fluency in Spanish required Proficient in computer systems and electronic health records Strong interpersonal and organizational skills Valid driver's license and reliable transportation (MVR check required) Work Environment

Community-based with daily travel required Moderate noise level; occasional exposure to outdoor conditions Flexibility to meet family needs, including crisis response Ready to make a difference in the lives of children and families?
