SUMMARY OF DUTIES: Freedmen’s Medicine is a hybrid medical practice model that specializes in transitional and in-home patient care for chronically ill patients. The Chief Medical Officer (CMO) is a transformative leader responsible for envisioning and delivering comprehensive clinical services at Freedmen’s Medicine. To be successful, the Chief Medical Officer (CMO) requires a comprehensive knowledge of chronic care management to include a comprehensive knowledge of chronic diseases such as chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, congestive heart failure, behavioral health, and substance abuse disorders. S/he works in collaboration and partnership with the Nurse Practitioners, the Clinical Director of Operations (DO), and the Care Management staff to maximize productivity, maximize billing, meet required deadlines, and ensure that critical patient issues are addressed in a timely manner.

PRIMARY RESPONSIBILITIES: This is a full-time position, Monday through Friday, which requires this position to see between 6-10 patients per day and therefore, requires the applicant to be well organized and proficient at time management. Provide direct day-to-day primary care, transitional care, and comprehensive health assessments to an assigned patient population in-home, in skilled care facilities and other locations within our assigned geographic area. Perform patient assessments, physical examinations, order/perform necessary laboratory and diagnostic tests, proficiency testing, prescribe and dispense medications in compliance with Freedmen’s established medical protocols. Complete documentation of client/patient findings, notes and recommendations at the end of each visit according to established 48-hour requirement. Review clinical care plans and participates in review of documentation and collaborates with the clinical and care management staff in the provision of care. Manages by providing leadership and specific direction to all patient care professionals and sets the standards of professional performance for medical and clinical support staff. Provides clinical supervision and mentorship to clinical staff and collaborates with the executive team to develop and implement strategies to improve patient outcomes and access to care. Lead multi-disciplinary team meetings and provide leadership to the clinical and care management teams. Works with members of the Freedmen’s management team to implement new clinic operating processes and systems (including EHR) to enhance patient flow, improve clinic administration and improve the overall patient experience. Assume responsibility for resolving patient incidents and reportable incidents, while providing as-needed consultation support to staff providers on issues relating to health care delivery. Regularly reviews and revises medical care policies and protocols as standards of care are revised and/or as directed by licensing, funding and program organizations. Performs regular medication review and reconciliation and regular chart reviews and audits on a monthly or quarterly basis as part of our ongoing Quality Assurance efforts. Works with the Clinical Director of Operations and the clinical staff to identify and operationalize appropriate key performance indicators and metrics to be used to assess clinical and care management performance.

EDUCATION: Must have a valid medical license in the states that Freedmens practices with no pending or previous disciplinary action, and must be board certified in a specialty

EXPERIENCE: A minimum of five years’ health care management experience required. Solid, proven managerial and administrative skills and expertise, preferably in environments featuring ethnically and socially diverse staff and clients.

ENVIRONMENTAL/ WORKING CONDITIONS: The Chief Medical Officer is required to work a combination of on-site location and virtually and therefore, will be required to physically be on-site as required.

PHYSICAL/ MENTAL DEMANDS: Must possess the cognitive/mental ability and patience to employ language and interactive communication skills while engaging with challenging patients suffering from physical, mental, and substance use disorders.
