HealthCare Support is actively seeking a
Senior UAS Assessor
to support members in
Onondaga County, NY . This organization is known for its commitment to high-quality, community‑based care management and its focus on accurate, timely assessments to support long‑term services and supports. In this role, the assessor will conduct comprehensive UAS‑NY assessments, collaborate with care teams, and help ensure members receive appropriate, high‑quality services—offering a meaningful position with direct impact on patient outcomes.
Schedule Monday – Friday, 8:30 AM – 5:00 PM
Compensation $90,000–$95,000/Annually
Responsibilities
Assess and coordinate medical and support services across the continuum of care for complex, high‑acuity populations to promote quality, cost‑effective outcomes.
Develop and manage personalized long‑term care plans while educating members and families on available services and benefits.
