Position Summary The Licensed Social Worker leads by example, promoting positive morale throughout the facility.
Works with residents, families, and team members to outline goals at admission, plan to meet those goals, and discharge as appropriate.
Responsibilities
Ensures resident adjustment to rehabilitation or long‑term care by performing psychological screenings, initial plan of care, introduction to services and procedures, and completing all necessary paperwork including MDS.
Reviews Resident Rights with all new admissions.
Ensures completion of admission agreements.
Collaborates with families, Business Office, and Medicare/Medicaid as liaison to assist residents with applications and qualification for sub‑acute/long‑term financial assistance.
Schedules interdisciplinary team meetings with medical staff, residents, and family members to plan care and resolve concerns; writes reports and ensures implementation.
Provides guidance and counseling to residents and families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
Coordinated discharge services – planning and acquisition of appropriate follow‑up care.
Qualifications
Long‑term experience: 1–3 years minimum.
Bachelor's Degree in Social Work, Psychology, or related field.
Social Work licensure.
