We are the torch bearers in South Central Pennsylvania's behavioral health, leading and inspiring others to their fullest potential. Our services are elite with our quality of care for the individuals and families we serve. We do this through our motto of “Passing Knowledge onto Others.”

Our consumers are adults ages 21 or older with autism spectrum disorder (ASD) and their families through the Pennsylvania State Program called the Adult Autism Waiver (AAW). The program is designed to help adults with ASD participate in their communities in their own way based upon their identified needs.

We aim to provide the best service to our consumers so that they may live to express their personal interest and healthy lifestyles.

O This business is based in Columbia Pennsylvania, but you would be driving from your home to meet with the clients in their homes and or community locations. The areas we serve are Adams, Berks, Cumberland, Dauphin, Franklin, Lancaster, Lebanon, Northumberland, Perry, and York.

We will pair you with clients that are within an approximate 45-minute commute or less.

Responsibilities Community Support Specialist (CS)

Facilitate the directive of the “Skill Building Plan” (SBP) from the Skill Builder Specialist (SB).

Assist participants in acquiring, retaining, and improving adaptive skills through coordinated goals attained from their ISP (Individual Support Plan) laid out by the SB.

Responsibilities

Facilitate social interaction and participation in community activities.

Improve capacity to perform daily living activities and instrumental activities (communication, cooking, housework, shopping, etc.) (in accordance with the SBP)

Support participants in managing their medical care, including scheduling and attending appointments, filling prescriptions, and monitoring health.

Collect data per session based on the criteria of the SBP and use a data collection sheet to communicate with the Skill Builder on progress.

Using “Jot Form,” report the activities and goals attainment of the participant from the session.

Report the participants visit start and ending time using “EVV” (Electronic Visit Verification) with the Sandata EVV Program.

Communication with teams regarding the participants’ activities as needed to:

Systematic Skill Builder (SSB)

Parents or Legal Guardians (Often)

Bureau of Autism Representatives (Rarely)

Requirements

High School Diploma or GED.

Willingness to travel to clients’ homes using your own reliable transportation with valid insurance.

Proficient with Microsoft Office products.

Mandated Person Check: FBI Clearance, Criminal Background.
