We are seeking a Hospital Liaison who serves as the primary connection between hospitals, physicians, and our long-term care facilities. This role focuses on building strong referral relationships, coordinating patient transitions, and promoting our services within the healthcare community.
Key Responsibilities
Develop and maintain relationships with hospital discharge planners, case managers, and physicians.
Educate healthcare professionals about our long-term care programs and services.
Coordinate admissions and ensure smooth transitions from hospital to facility.
Conduct on-site hospital visits, presentations, and community outreach events.
Track referral activity and maintain accurate records in CRM systems.
Collaborate with internal teams to meet census and occupancy goals.
Qualifications
Bachelor’s degree in healthcare, marketing, or related field (preferred).
Minimum 2 years of experience in healthcare marketing, hospital liaison, or admissions (long-term care experience strongly preferred).
Strong communication, relationship‑building, and organizational skills.
Knowledge of Medicare, Medicaid, and insurance processes.
Ability to travel locally and work flexible hours as needed.
