Above and Beyond Monticello is seeking a compassionate and dedicated Home Care Primary Care /

Registered Nurse (RN)

to join our home health team. In this role, you’ll provide high-quality, patient-centered care in the comfort of patients’ homes, helping them maintain their health, independence, and dignity.

Coverage area is approximately between Cedar Rapids and Monticello.

Zero On-Call Requirements

– focus on care, not being on call

Key Responsibilities

Conduct comprehensive assessments of patients’ health status and needs

Develop and implement individualized care plans in collaboration with interdisciplinary teams

Administer medications and treatments, monitor patient responses

Educate patients and families on disease management and health maintenance

Monitor vital signs and manage chronic conditions

Accurately document patient care activities

Coordinate care with physicians, therapists, and other healthcare professionals

Provide emotional support and counseling to patients and families

Ensure compliance with healthcare regulations and standards

Participate in quality improvement initiatives and ongoing education

What We’re Looking For

Graduate of an approved nursing program (BSN preferred).

Current RN license in the state of practice.

At least

1 year of professional nursing experience

(Home health experience is a plus!).

Strong computer and technology skills.

A valid driver’s license and reliable transportation.
